For more than twenty years, I accompanied patients in their rehabilitation journeys. After a stroke, a fracture, or complex surgery, we built rehabilitation programs together. Progress was visible: steps steadier, hands more flexible, confidence regained. But at every hospital discharge, one question always came up: who takes over?

Often, families received a file, a few recommendations, and the promise of follow-up. But once home, it was another story. Care assistants were present but not always well-informed. Community health professionals did their best, each in their own domain. Family caregivers improvised. And clinical progress risked slipping away.

This is the gap I wanted to fill. By becoming an autonomy coordinator, I chose to connect what was disconnected. My role is no longer about performing medical acts, but about ensuring continuity. Concretely, this means organizing the work of care assistants, securing schedules, installing teleassistance as a permanent safety net, and making sure information flows.

With Jamacare, we do not simply stack services together. We build an integrated system. Care assistants are not left alone, caregivers know where to find support, families sleep more peacefully. Autonomy stops being an uncertain gamble — it becomes a structured project.
