Redefining Home Autonomy: How my Perspective as a Former MPR Doctor Transforms the Home into a Therapeutic Lever.
Drawing upon my background as a former doctor and head of department in Geriatric Physical Medicine and Rehabilitation (PMR/MPR), I have dedicated decades to analyzing the complex interdependence between living environments and an individual’s ability to regain or maintain their autonomy. Today, in my role as an autonomy coordinator, my intervention extends far beyond simple assessment or physical adaptation of a dwelling. I perform a genuine « home diagnostic »: a multidimensional and strategic analysis that integrates all clinical, functional, and psycho-social factors of the patient.
This is no longer just about identifying physical obstacles like door thresholds, slippery rugs, or the absence of grab bars. My approach involves deeply understanding how the specific nature of each pathology – be it chronic heart failure leading to increased fatigue, complex neurological sequelae from a stroke, the progressive evolution of a neurodegenerative disease like Parkinson’s or Alzheimer’s, or sensory disorders affecting proprioception – impacts every gesture, every movement, and every moment of daily life at home. I focus on unnecessary efforts that can exhaust a fragile patient, increased fall risks linked to a combination of visual and balance impairments, or the detrimental effect of an unstimulating environment on morale and cognitive functions.
My clinical experience provides me with a unique ability to anticipate future needs, identify subtle signs that could lead to deterioration, prevent costly and often traumatic rehospitalizations, and optimize the use of the patient’s residual capacities. I collaborate closely with occupational therapists, recognized experts in environmental modifications and assistive devices, bringing my sharp clinical perspective to their recommendations. My role is to refine their suggestions to ensure that every piece of equipment, every architectural modification, and every support service seamlessly integrates into a comprehensive, coherent, and evolving life and care plan. It is this holistic analysis, this strategic medical perspective forged by years of practice, that generates invaluable worth for the patient, their caregivers, and all professionals involved.
