From Clinic to Daily Life: My Role as a Former MPR Doctor for a Successful, Secure, and Deeply Human Hospital Transition.

Returning home after a period of hospitalization is a critical transition, often generating significant anxiety and a sense of uncertainty for both the patient and their loved ones. Drawing upon my exceptional professional journey as a former doctor and head of department in Geriatric Physical Medicine and Rehabilitation, I have had the privilege of orchestrating countless hospital discharges, which has provided me with an intimate understanding of the challenges, expectations, and pitfalls of this delicate period.

My mission as an autonomy coordinator is precisely to build an unwavering bridge between the highly medicalized environment of the healthcare institution and the complex, often less structured reality of the home. I am perfectly fluent in the language, protocols, and requirements of hospital medical and paramedical teams. This skill allows me to communicate effectively with them, gather clinical information of unparalleled precision regarding the initial diagnosis, short- and long-term functional prognoses, ongoing medication treatments and their potential side effects, as well as the patient’s specific post-hospital rehabilitation needs.

This in-depth knowledge and communication capability allow me to acutely anticipate needs for assistive devices (whether adjustable medical beds, wheelchairs adapted to posture and mobility, or specific bathing aids), to smoothly organize and coordinate liaison with home care services, liberal healthcare professionals (nurses, physiotherapists, speech therapists), and to proactively adapt the home environment, even before the patient’s actual return. My expertise as a former doctor guarantees that no detail is overlooked, drastically minimizing the risks of early rehospitalization and ensuring a return home that is not only safe and comfortable but also conducive to continued rehabilitation, maintenance of autonomy, and progressive improvement in quality of life. I am the guarantor of a transition that is not only fluid and organized but also deeply human and confidence-inspiring for a serene new beginning at home.
