CHRISTOPHE DELONG Coordinateur d’Autonomie

Votre autonomie, notre expertise Aide à domicile premium avec coordination personnalisée et gestion totale des démarches dans l’Ouest Parisien

Daily life can be complex, especially when fragility or illness alters our ability to act and decide. Prevention protects us before problems arise, curative care treats diseases, and palliative care supports those who can no longer be cured. But how can we guide someone through these stages so that life remains meaningful and autonomous? This is where rehabilitation comes in. According to WHO, it optimizes functioning and reduces disability, creating the essential link between care and daily life.

I am Christophe Delong, autonomy coordinator at Jamacare. In my role, I become the GPS for vulnerable people—elderly adults, disabled adults, or children needing specific support. Every journey is unique: some regain lost movements, others relearn how to move or organize daily life. But this GPS is never solitary. Caregivers, social workers, physiotherapists, and sometimes neighbors or family members participate in this complex navigation. Together, we choose the best routes so that the person reaches life goals and maintains autonomy.

Rehabilitation is thus a dynamic mapping of life. It does not merely correct motor or cognitive deficits; it coordinates, anticipates, and adjusts pathways according to the person’s needs and environment. It integrates prevention, curative, and palliative care so that every stage of life is supported, safe, and meaningful.

Every day, I see this GPS work when every actor understands their mission and collaborates fully. Rehabilitation becomes a strategic and human tool that transforms vulnerability into autonomy and dependency into social participation.

With Jamacare, discover how rehabilitation can become your GPS to autonomy and a meaningful life.
