The change ahead is historical. As the Autonomy Coordination Manager, my role is to ensure the administrative logistics of the major reform set to take effect in December are perfectly understood and anticipated by the team. The inequitable system that led to massive out-of-pocket expenses is over. The National Health Insurance (Assurance Maladie or AM) now covers the actual cost of wheelchairs (FR), but to regulate this national expense, it introduces the Maximum Selling Price (Prix Limite de Vente – PLV). It is fundamental for our home helpers and Social Workers to understand that the PLV is the new financial anchor securing the commitment of the ONDAM (National Health Insurance Spending Target).

 

Logistical Consequences and the Risk of Standardization

 

The optimistic scenario is powerful: administrative simplification and financial security. For Social Workers, the FR’s base funding is secured, significantly simplifying complex procedures and freeing up time to focus expertise on human assistance (PCH) and critical adaptations. For beneficiaries, debt anxiety ends. My former PMR expertise allows me to assert that this financial guarantee promotes better acceptance of prescribed equipment.

However, as a care facilitator, it is my duty to anticipate the pessimistic scenario. Excessive PLV pressure on manufacturers could lead to standardization or the exclusion of crucial fine-tuning options (complex seating, lightweight chassis). For home helpers, this could mean maneuvering less ergonomic or less suitable FRs for transfers, increasing the physical difficulty of their work. This is unacceptable. While I hold no hierarchical role over prescribers, I am responsible for facilitating information flow. We must inform prescribing caregivers (PMR specialists, etc.) about the logistical risks linked to suboptimal FRs. Our home helpers are best positioned to alert us to daily use problems, allowing Social Workers to document the need for complementary funding.
